Summary

As the Home Visitor you will provide effective home-based, family-centered services to families in compliance with Greater Seacoast Community Health (GSCH) policies and state contracts. The population to be served is expectant women and families with children up to the age of 21. The Home Visitor will help families develop Individualized Family Support Plans and document activities for billing and case records. The Home Visitor will coordinate services with GSCH staff, other social services, and medical practices.

Duties

  • Completes required documentation for each referral and family file in accordance with GSCH policy and contract requirements.
  • Engages each family by establishing rapport using a trauma informed approach.
  • Maintains confidentiality and ethical boundaries in accordance with GSCH policies and contract requirements.
  • Develops an Individualized Family Support Plan (IFSP) which identifies the family's strengths and challenges while creating goals towards targeted support, advocacy, and education.
  • Maintains detailed notes on all activities, in a timely manner, when working with families as required by GSCH policy, contract requirements, and New Hampshire rules.
  • Assesses strengths and concerns of each family using a variety of assessment tools, such as Protective Factors Surveys and ASQs.
  • Works collaboratively and coordinates services for each family to other internal GSCH and/or external community resources.
  • Attends necessary meetings and regular supervision.
  • Attends trainings as assigned and stays current on required certifications, such as Family Resource Center of Quality Standards training.
  • Completes all other required tasks as assigned by manager.

Requirements

  • Associate's degree in social work, counseling, human services, early childhood education, or related field.
  • At least 2 years' experience working with families and children in social service, home health, or early childhood program settings.
  • Must be able to utilize a strength-based approach when engaging with families. (The Home Visitor will be trained in the Strengthening Families Protective Factors Framework and stay current with the Family Centered of Quality Standards training.)
  • Knowledge of case management and family support principals required.
  • Strong computer, communication, teaching, and problem-solving skills needed.
  • Must demonstrate an understanding of confidentiality, boundaries, and engage in ethical practice.
  • Must be a well-organized, detail oriented, approachable, flexible professional who is able to work independently and in collaboration with others.
